Insurance & Financial Refund Policy
As a participating provider for most insurances, we will verify and authorize your insurance 2 days prior to your appointment, along with submitting the proper claim after your visit. If not verified prior, we will collect at time of checking in for your appointment.
If we do not participate with your insurance, we will provide you with the proper receipt for you to file your insurance claim.
PATIENT'S RESPONSIBILITY IS TO OBTAIN ANY REFERRALS BEFORE THE SCHEDULED APPOINTMENT; if stated by your insurance carrier. If we do not have the appropriate referral and or authorization before the appointment, you may opt to reschedule or self pay.

*Please be ready with your vision and or medical insurance card, we will need it.
*If you have a Medicare *Replacement plan, we will need that information from your card.
*You can call the number on your insurance card to verify if we participate with your plan.
